Privacy Policy

The 4th Angel Mentoring Program is a complimentary mentoring program provided by The Cleveland Clinic Foundation Tausig Cancer Institute (the “Mentoring Program”). Our Privacy Policy describes how we handle the information that you provide to us when you use the website www.4thangel.ccf.org (the “Website”) and participate in the Mentoring Program through the Website (collectively, the “Services”). You understand that through the use of the Services you consent to the collection and use of this information as set forth in this Privacy Policy.

This Privacy Policy does not govern or apply to information collected or used by us through other means. This Privacy Policy also does not apply to the practices of companies that we do not own or control, or to individuals that we do not employ or manage.

This Privacy Policy covers the following:

INFORMATION COLLECTED AND USE.

User account registration information. If you choose to register for an account to use the Services, then we collect the following personal information provided by you: your first and last name, user name, email address, and date of birth. In addition to using the information you provide to register your account, your user name and age (through date of birth) are used in your profile. We may also use this information to contact you about the Services, to respond to your emails or online requests about the Services, or for us or Cleveland Clinic to contact you to participate in surveys related to patient care.

Registered user additional information. Once you have registered to use the Services, you may voluntarily provide, but are not required to do so, other personal information such as: phone number, gender, address, relationship status, sexual orientation, race, ethnicity, religion, employment status, language, location (city/state), type of cancer and stage, and diagnosis. The information you voluntarily disclose is used to match you with a mentor or mentee, depending on how you have registered to use the Services.

Your account and profile information are not displayed to the public on the Website. If you are a registered mentee, then when you are logged in to use the Services you are able to view the user name and age and, if provided, gender, diagnosis, location, and strength of match of potential mentors. A registered mentor logged in to use the Services will only see the information of a mentee that such mentee has chosen to disclose in their profile if and when the mentee makes a match request.

Non-Personal Information Collected Automatically. When you visit the Website or when you are using the Services, we automatically receive and record non-personal information on our server logs from your browser, including your IP address (which can be used to determine your Internet Service Provider and geographic location or point of connectivity), cookie information, and the page you requested. The information that is automatically collected is used to manage the Services, track usage, understand user behavior, and improve the Services.

First Party Cookies. We collect information using "first party cookies”, which are alphanumeric identifiers that we transfer to your computer's hard drive through your web browser. Cookies are never associated with specific personal identities. First party cookies are distinct from third party cookies that they are created and directly served by the company hosting the website.

We use two types of “cookies” on the Website:

  • We use persistent cookies to recognize a repeat visitor, enabling us the opportunity to offer the visitor a set of services or information requested in a previous visit.
  • We use session cookies to track a visitor's path through our site during a visit, to help us understand how people use our site.

INFORMATION SHARING AND DISCLOSURE.

We will not sell or rent your information to anyone. We will not share personally identifiable information of any individual with any third party unrelated to us, except for legal purposes or investigations, or if so directed by an individual through a proper authorization.

We may share non-identifying information (type of cancer and age only) about a user with a partner organization if we are unable to match such user with a mentor and for purposes of determining whether the partner organization would be able to provide a mentor match. If a partner organization has a match, we will contact the user and inform them that they may contact such partner organization if they are interested.

We may view any activity that occurs through your use of the Services. We will only view your activity in order to help you resolve usability issues, in situations where you have made or are the subject of a report in order to investigate such situations, you have blocked a user or have been blocked by a user in order to investigate such situations, or if we believe you have violated a term of the Agreements (as that term is defined in our Terms of Use).

USE AND DISCLOSURE OF HEALTH INFORMATION.

You acknowledge that you have received Cleveland Clinic Health System’s Notice of Privacy Practices. The Notice of Privacy Practices explains how we may use and disclose confidential health information that identifies you. You consent to let us use and disclose health information about you as described in the Notice of Privacy Practices. In doing so you consent to the release of your health information and financial account information to all third-party payers and/or their agents that are identified by us, our billing agents, collection agents, attorneys, consultants, and/or other agents that represent us or provide assistance to us for the purposes of securing payment from all parties who are potentially liable for payment for your health care, including for substance abuse, psychiatric care, or HIV, if applicable. You can revoke your consent in writing at any time except to the extent that we have already relied on your consent.

YOUR ABILITY TO ACCESS, EDIT, AND DELETE YOUR INFORMATION AND DEACTIVATE YOUR ACCOUNT.

You can access your account and profile information at any time. You may edit, update, modify, or delete your profile information at any time. You may also deactivate your account at any time, as explained in our Terms of Use.

SECURITY.

Your account is protected by a password for your privacy and security. You need to prevent unauthorized access to your account and personal information by selecting and protecting your password appropriately and limiting access to your computer and browser by signing off after you have finished accessing your account.

We are committed to protecting your information and ensuring that your account information is kept private, however, we cannot guarantee the security of your account information. Unauthorized entry or use, hardware or software failure, and other factors, may compromise the security of user information at any time. In using the Services, you acknowledge that breaches in security may occur from time to time, and hold us harmless if they do.

CHANGES TO THIS PRIVACY POLICY.

We may modify this Privacy Policy at any time without notice. The modified privacy policy will be effective upon posting on our Website. To remain in compliance, we suggests that you review the Privacy Policy at regular intervals.
